Testimonial Release Form

I, the undersigned, hereby grant Innovative Language, its directors, officers, employees, designees, agents, independent contractors, legal representatives, successors and assigns the worldwide, irrevocable, and royalty free right to record, copy, distribute, display, publish, and use my image, voice, likeness, testimonial, biographical data, and persona in and through any media in existence or yet to be invented (including print, digital, electronic, broadcast, or otherwise) for any purpose whatsoever, including publicity and advertising.

I grant Innovative Language the right to capture my image, voice, likeness, identity, and testimonial through audio, video, print, or any other means in existence. I grant Innovative Language the right to amend, modify, edit, abridge, and change any image, audio, video, testimonial, data, or other information captured or submitted to Innovative Language.

I hereby waive any right that I may have to inspect and approve the finished product used or to which my testimonial may be applied.

I release and agree to hold harmless Innovative Language, its directors, officers, faculty, agents, of and from any liability by virtue of capturing my image, voice, likeness, identity, or testimonial or using the testimonial or biographical data, in any manner or for any reason whatsoever.

I understand that I will receive no compensation for my consent and that all information, images, audio, video, and data will become the property of Innovative Language.

If such testimonial is being given by a minor, then verify that I am the parent of such minor and have the authority to grant the rights above and do hereby grant such rights on behalf of my child.
